myHealthCard - Side 2 contains the following info: patient name, address, phone number, height, usual weight, allergy info, medical contact info (names & numbers of healthcare providers and pharmacy), and emergency contact info. Space permitting, other info may also be placed here (such as past medical history, past surgeries, etc.) upon request.
